Telehealth Consent & Disclaimer

By scheduling or participating in a telehealth visit with Sageview Medical, you acknowledge and consent to the following:
Nature of Telehealth: You understand that telehealth involves the electronic transmission of health information and may include video, audio, and/or data communication.
Limitations: Telehealth may not be equivalent to in-person care and may be subject to technical disruptions, poor connectivity, or incomplete evaluation.
Licensing: You agree that you are physically located in the State of West Virginia (or another state where our providers are duly licensed) at the time of the telehealth encounter.
No Guarantee of Treatment: You acknowledge that the provider may determine telehealth is not appropriate for your condition and may recommend in-person evaluation or referral.
Risks: Potential risks include, but are not limited to, equipment failure, incomplete data transmission, or inadvertent disclosure of information due to security vulnerabilities.

Cancellation & Refund Policy
Effective Date: August 4, 2025
Cancellations & Rescheduling:
Appointments may be cancelled or rescheduled with at least 24 hours’ notice. Failure to provide timely notice may result in a cancellation fee.
Refunds:
Prepaid services, including IV hydration treatments, injections, and wellness packages, are non-refundable once treatment has commenced.
If Sageview Medical cancels a service for any reason, patients will be entitled to a full refund or rescheduling at no additional charge.
Refunds, when applicable, will be processed to the original form of payment within 10 business days.
No-Shows:
Failure to attend a scheduled appointment without prior cancellation may result in forfeiture of prepaid services.
Shipping Policy
Most of our services are provided in person, either through telehealth visits or at-home/mobile IV hydration and wellness care, and do not involve shipping. For compounded prescriptions billed by Sageview Medical, medications are prepared and shipped securely through licensed compounding pharmacies to your designated address. Once shipped, these items cannot be canceled or refunded. Prescriptions sent to retail pharmacies are handled, billed, and shipped directly by the pharmacy.
